When this coupon code is used, we will email you a prepaid shipping label to your email. Please ship teeth models back padded and protected to withstand the rigors of shipping.

If you have placed your custom mouth guard order with Damage Control September 2017 or after, then we have scanned your teeth, and we can 3D print your teeth models for fabrication. Just place your order as you normally would from our custom mouth guard or VFP product page and use coupon code: 3Dprintmyteeth for $15 OFF*
*Note impression kits will not be shipped if any of these coupon codes are used. The only thing that will be shipped is your completed custom mouth guard after we 3D print your teeth models or receive your teeth models in the mail.
